The Ultimate UFC Sweepstakes

OFFICIAL RULES

NO PURCHASE IS NECESSARY TO ENTER OR WIN. A PURCHASE DOES NOT INCREASE THE CHANCES OF WINNING.

1. Sponsor: The Sponsor of The Ultimate UFC Sweepstakes (the "Sweepstakes") is My Medic, located at 120 North Redwood Road, North Salt Lake, UT 84054 (the "Sponsor").

2. Eligibility: The Sweepstakes is open only to legal residents of the 50 United States and the District of Columbia who are 18 years of age or older at the time of entry. (Note: Please ensure age 18 meets all specific requirements for hotel check-in and event attendance related to the Grand Prize.) The Sweepstakes is void where prohibited by law. Employees, independent contractors, interns, officers, directors, and agents of My Medic (the "Sponsor"), Zuffa, LLC (d/b/a UFC), and ESPN, Inc. (and their respective affiliates, subsidiaries, advertising and promotion agencies, suppliers – collectively, "Sweepstakes Entities"), and their immediate family members (spouse, parents, siblings, children) and/or those living in the same household of each are not eligible to participate in the Sweepstakes. The Sweepstakes is subject to all applicable federal, state, and local laws and regulations. Void where prohibited.

3. Agreement to Rules: By entering the Sweepstakes, the contestant ("You") agree to comply with and abide by these Official Rules and the decisions of the Sponsor, which are final and binding in all respects. You represent and warrant that You meet the eligibility requirements. In addition, You agree to accept the Sponsor's decisions as final and binding as they relate to the content and administration of this Sweepstakes. Entrants should look solely to the Sponsor with any questions, comments, or issues related to the Sweepstakes.

4. Entry Period: The Sweepstakes begins on May 20, 2025 at 12:00 PM MDT (Noon Mountain Time) and ends on June 15, 2025 at 12:00 PM MDT (Noon Mountain Time) (the "Entry Period"). Entries submitted before or after the Entry Period will not be eligible. The Sponsor’s computer is the official time-keeping device for the Sweepstakes.

5. How to Enter: The Sweepstakes must be entered by submitting an entry using the online form provided at https://mymedic.com/pages/ufc-sweep-stakes. The entry must fulfill all requirements of the Sweepstakes, as specified, to be eligible to win a prize. Entries that are incomplete or do not adhere to the rules or specifications of the Sweepstakes may be disqualified at the sole discretion of the Sponsor. You must provide the information requested (e.g., name, email address). Limit one (1) entry per person/email address. You may not enter more times than indicated by using multiple email addresses, identities, or devices in an attempt to circumvent the rules. If You use fraudulent methods or otherwise attempt to circumvent the rules of the Sweepstakes, your submission may be disqualified at the sole discretion of the Sponsor.

6. Prizes: There will be one (1) Grand Prize Winner and fifty (50) Runner-Up Winners selected.

• Grand Prize (1 Winner): The Grand Prize Winner will receive a prize package consisting of:

• Two (2) lower level tickets to UFC® 317 scheduled to take place in Las Vegas, Nevada on June 28, 2025. (Approximate Retail Value ("ARV") of tickets: $2000 USD). Tickets are subject to terms and conditions specified thereon. Seat locations are determined by Sponsor in its sole discretion.

• Travel Credit: A travel credit or voucher(s) intended for round-trip coach class air transportation and/or hotel accommodation related to attending the event. (Total ARV: $750 USD). Winner may allocate the $750 credit towards flights and/or hotel stays related to the prize trip at their discretion, but is solely responsible for booking arrangements and any costs exceeding the $750 credit value. Travel restrictions, booking requirements, and other conditions may apply.

• My Medic Gear: A selection of My Medic products, such as a MyFAK Large Pro and a Bundle. (ARV: $548.55 USD).

• Total Grand Prize ARV: $3298.55 USD.

• Runner-Up Prizes (50 Winners): Fifty (50) Pay-Per-View Codes (“PPV Code”) will be awarded to each selected Runner-Up winner to watch UFC Fight 317 on Pay-Per-View (each, a “Runner-Up Prize”). PPV Code is valid only for U.S. residents, 18 years of age or older. PPV Code entitles the holder to the UFC Fight 317 PPV Event on ESPN+, which will be credited to the Runner-Up Winner’s ESPN account upon redemption. A condition to redemption is that the holder must accept the ESPN+ Subscriber Agreement in connection with their access to ESPN+. PPV Code is one-time use only. Void where prohibited, not a gift card. Cannot be combined with any other offers, coupons, discounts or promotions. PPV Code is not refundable or redeemable for cash or any other goods or services and cannot be sold, or exchanged or credited towards purchase of other subscriptions, and will not be honored if obtained from any third party. (ARV of each PPV code: $80 USD).

• Total ARV of all Runner-Up Prizes: $4000 USD.

General Prize Conditions: The ARV of prizes may differ at the time of prize award. The specifics of the prizes (including, but not limited to, ticket locations, airline, flight times, hotel selection) shall be solely determined by the Sponsor. No cash or other prize substitution shall be permitted except at the Sponsor's discretion. Prizes are non-transferable. Substitution of the prize or transfer/assignment of the prize to others or request for the cash equivalent of the prize by the Winner is not permitted. Any and all prize-related expenses, including without limitation any and all federal, state, and/or local taxes, ground transportation, meals, incidentals, gratuities, baggage fees, and any other expenses not specifically listed herein shall be the sole responsibility of the Winner. The Grand Prize Winner may be required to provide a credit card at hotel check-in for incidental charges. The Grand Prize Winner and guest must travel on the same itinerary and comply with all venue, airline, and hotel requirements, including any health and safety protocols. The Grand Prize Winner's guest must be of the age of majority in their jurisdiction of residence unless the Winner is the parent/legal guardian of the guest. Acceptance of the prize constitutes permission for the Sponsor to use the Winner’s name, likeness, and entry for purposes of advertising and trade without further compensation, unless prohibited by law. Grand Prize Winner may be required to complete and return an Affidavit of Eligibility, Liability Release, and Publicity Release (where lawful) within 5 days of notification.

7. Odds of Winning: The odds of winning depend on the number of eligible entries received.

8. Winner Selection and Notification: One (1) Grand Prize Winner and fifty (50) Runner-Up Winners will be selected in a random drawing from all eligible entries received during the Entry Period. The drawing will be conducted on or about June 16, 2025, under the supervision of the Sponsor. Winners will be notified via email on or about June 16, 2025. The Sponsor shall have no liability for a Winner's failure to receive notices due to spam, junk e-mail, or other security settings or for a Winner’s provision of incorrect or otherwise non-functioning contact information. If a selected Winner cannot be contacted, is ineligible, fails to claim the prize within seven (7) days from the time award notification was sent, or fails to timely return a completed and executed declaration and release as required (i.e., within 5 days as specified in Section 6), the prize may be forfeited and an alternate Winner selected from the remaining eligible entries. Receipt of the prize offered in this Sweepstakes by the Winner is conditioned upon compliance with any and all federal, state, and local laws and regulations, and these Official Rules. ANY VIOLATION OF THESE OFFICIAL RULES BY A WINNER (AT THE SPONSOR'S SOLE DISCRETION) WILL RESULT IN SUCH WINNER'S DISQUALIFICATION AS WINNER OF THE SWEEPSTAKES, AND ALL PRIVILEGES AS WINNER WILL BE IMMEDIATELY TERMINATED.

9. Rights Granted by the Entrant: By entering this Sweepstakes, You understand and agree that the Sponsor and anyone acting on behalf of the Sponsor or its respective licensees, successors, and assigns, shall have the right, where permitted by law, to print, publish, broadcast, distribute, and use in any media now known or hereafter developed, in perpetuity and throughout the World, without limitation, your entry, name, portrait, picture, voice, likeness, image, statements about the Sweepstakes, and biographical information for news, publicity, information, trade, advertising, public relations, and promotional purposes without any further compensation, notice, review, or consent. By entering this content, you represent and warrant that your entry is an original work of authorship, and does not violate any third party’s proprietary or intellectual property rights. If your entry infringes upon the intellectual property right of another, you will be disqualified at the sole discretion of the Sponsor. If the content of your entry is claimed to constitute infringement of any proprietary or intellectual proprietary rights of any third party, you shall, at your sole expense, defend or settle against such claims. You shall indemnify, defend, and hold harmless My Medic from and against any suit, proceeding, claims, liability, loss, damage, costs or expense, which My Medic may incur, suffer, or be required to pay arising out of such infringement or suspected infringement of any third party’s right.

10. Terms and Conditions: The Sponsor reserves the right, in its sole discretion, to cancel, terminate, modify or suspend the Sweepstakes should virus, bug, non-authorized human intervention, fraud, or other cause beyond the Sponsor’s control corrupt or affect the administration, security, fairness, or proper conduct of the Sweepstakes. In such cases, the Sponsor may select winners from eligible entries received before and/or after the action taken by the Sponsor if appropriate. The Sponsor reserves the right at its sole discretion to disqualify any individual who tampers or attempts to tamper with the entry process or the operation of the Sweepstakes or website or violates these Terms & Conditions. The Sponsor has the right, in its sole discretion, to maintain the integrity of the Sweepstakes, to void entries for any reason, including, but not limited to: multiple entries from the same user from different IP addresses; multiple entries from the same computer in excess of that allowed by Sweepstakes rules; or the use of bots, macros, scripts, or other technical means for entering. Any attempt by an entrant to deliberately damage any website or undermine the legitimate operation of the Sweepstakes may be a violation of criminal and civil laws. Should such an attempt be made, the Sponsor reserves the right to seek damages to the fullest extent permitted by law.

11. Limitation of Liability: By entering the Sweepstakes You agree to release and hold harmless My Medic, Zuffa, LLC (d/b/a UFC), ESPN, Inc., and their respective subsidiaries, affiliates, advertising and promotion agencies, partners, representatives, agents, successors, assigns, employees, officers and directors (collectively, the "Released Parties") from any liability, illness, injury, death, loss, litigation, claim or damage that may occur, directly or indirectly, whether caused by negligence or not, from (i) such entrant’s participation in the Sweepstakes and/or his/her acceptance, possession, use, or misuse of any prize or any portion thereof; (ii) unauthorized human intervention in any part of the Sweepstakes; (iii) electronic or human error in the administration of the Sweepstakes or the processing of entries; (iv) technical errors of any kind, including but not limited to the malfunction of any computer, cable, network, hardware, or software, or other mechanical equipment; (v) the unavailability or inaccessibility of any transmissions, telephone, or Internet service; (vi) printing errors; (vii) lost, late, postage due, misdirected, or undeliverable mail or email; or (viii) errors in the announcement of the prizes or winners; or (ix) travel to or participation in any prize-related activity, including the UFC event.

12. Disputes: THIS SWEEPSTAKES IS GOVERNED BY THE LAWS OF THE UNITED STATES AND THE STATE OF UTAH, WITHOUT RESPECT TO CONFLICT OF LAW DOCTRINES. As a condition of participating in this Sweepstakes, You agree that any and all disputes that cannot be resolved between the parties, and causes of action arising out of or connected with this Sweepstakes, shall be resolved individually, without resort to any form of class action, exclusively before a court located in Utah having jurisdiction. Further, in any such dispute, under no circumstances shall You be permitted to obtain awards for, and You hereby waive all rights to, punitive, incidental, or consequential damages, including reasonable attorney’s fees, other than Your actual out-of-pocket expenses (i.e. costs associated with entering the Sweepstakes). You further waive all rights to have damages multiplied or increased.

14. Winners List: To request a copy of the winners list (available after July 16, 2025), please contact the Sponsor by mail at 120 North Redwood Road, North Salt Lake, UT 84054. Requests must be received no later than four (4) weeks after the end of the Entry Period.

15. Non-Sponsorship Disclaimer & Trademarks: This Sweepstakes is solely sponsored by My Medic. Zuffa, LLC (d/b/a UFC) and ESPN, Inc. are prize providers only and are not sponsors of this Sweepstakes and are not responsible for its administration or execution. This Sweepstakes is in no way sponsored, endorsed, administered by, or associated with Facebook, Twitter, Instagram, Snapchat, YouTube, Reddit, Pinterest, LinkedIn or any other social networks that may be used to share the Sweepstakes. You understand that you are providing your information to the Sponsor (My Medic) and not to Zuffa, LLC, ESPN, Inc., or any social media platform. UFC®, ZUFFA®, and related marks are registered trademarks of Zuffa, LLC. ESPN®, ESPN+®, and related marks are registered trademarks of ESPN, Inc. My Medic® and related marks are registered trademarks of SLED distribution.

16. Acceptance of Rules: By participating in the Sweepstakes, You have affirmatively reviewed, accepted, and agreed to all of the Official Rules, Terms and Conditions.
